Source text - English The genetic architecture of Autism Spectrum Disorder
Before summarizing epidemiological and molecular genetic studies of Autism Spectrum Disorder (ASD), it is important to consider our current understanding of human genome diversity.
An individual carries, on average, 3 million genetic variants that differ from the reference human genome sequence. Approximately 130,000 of the variants (~4% of the genome) are shared with only 0.5–5% of the population (these are known as rare variants), and less than 1% of the variants are either unique to the individual or shared by a very small number of relatives.
Remarkably, the range of mutations that affect protein sequences is considerably different today from the range that existed as recently as 200 to 400 generations ago. It is estimated that each individual carries, on average, 40–110 variants that are classified by the Human Gene Mutation Database as disease-causing mutations.
Thus, carrying a disease mutation is not the exception but the rule. The central question for epidemiologists and molecular geneticists is therefore: how are low-, medium- and high-risk mutations distributed in the population and how can certain individuals remain unaffected despite carrying deleteri¬ous ones?
The genetic architecture of ASD is heterogeneous. In one subset of patients, a single fully penetrant mutation can be enough to cause ASD, but in another an accumulation of many low-risk alleles could be the origin of this disorder.
Epidemiology and heritability of ASD
Social skills and general intelligence, as measured by psychological and cognitive tests, are quantitative traits that show a normal distribution in the general population. Individuals diagnosed with ASD or ID are considered to be at the lower end of this distribution. However, the demarcation between affected and non-affected indi¬viduals based on these traits is difficult to draw.
In addition, although people with clinical ASD and indi¬viduals with autistic-like traits differ from each other in the severity and/or degree of functional impairment, twin studies have shown that they could share the same susceptibility genes.
The relative influence of genetic, epigenetic and environmental factors on the risk of ASD remains under debate, but all epidemiological studies point to there being a strong genetic contribution to the risk of ASD. In all twin studies conducted to date on ASD, monozygotic twins have higher concordance rates for ASD (ranging from 60% to 90%) than dizygotic twins have (from 0% to 30%).

Antes de resumir los estudios epidemiológicos y genéticos sobre los Trastornos del Espectro Autista (TAE), es importante tener en cuenta los conocimientos actuales sobre la diversidad del genoma humano.
Cada individuo tiene, en promedio, 3 millones de variantes genéticas que difieren de la secuencia del genoma humano de referencia. Source text - English The reprocessing instructions have been validated to ensure the sterility of the reusable SURGINST system surgical instruments in their sterilization trays.
It is the responsibility of the reprocessing facility to ensure that reprocessing is performed using the appropriate equipment and materials and that personnel have been properly trained in order to achieve the desired cleaning and sterilization.
It is required that equipment and processes are validated and routinely monitored.
Any deviation by the reprocessing facility from these instructions should be evaluated for effectiveness to avoid potential adverse consequences.
It is essential that the surgeon and operating room staff are fully familiar with the SURGINST surgical technique.
The instruments should be placed in their dedicated slots inside the container during transportation, after surgery and throughout the cleaning process to avoid damage or improper treatment.
Warnings and Precautions
Reprocessing should begin with processing at the point of use (i.e., close proximity to the point of use of the device), to facilitate subsequent cleaning steps.
Surgical instruments are supplied non-sterile and have to be cleaned, disinfected and sterilized according to the following instruction before use:
Inspection, Functional Testing and Maintenance
Following cleaning, thoroughly inspect the items, including sterilization case and tray, for cleanliness.
All visible surfaces, internal and external, should be visually inspected for residue biologic soil.
If necessary, re-clean the instrument until it is visibly clean.
Visually inspect each instrument (including sterilization case and tray) for damage, wear, cracks, or corrosion.
If damage or wear is observed that might compromise the function of the device, do not process it further and contact your representative for a replacement.
When inspecting, check the following:
Movable parts (hinges, joints, sliding parts, movable features) should operate smoothly throughout the intended range of motion.
Packaging
Place the instruments inside the sterilization case.
Ensure that the sterilization case total weight (i.e., with the instruments within it) does not exceed 10 kg.
Wrap the trays using a delinted textile double wrapper, which is intended for sterilization tray wrapping and which was inspected prior to use.
The package must permit penetration and contact of the sterilant with the items (as well as air removal and steam evacuation), while also serving as a barrier to microorganisms, during any storage period.
Sterilization
SURGINST System Surgical Instruments should be steam sterilized.
The autoclave manufacturer's operating instructions and recommended guidelines for maximum sterilization load should be followed.
The autoclave must be properly installed, maintained and calibrated.

Source text - English Process Validation and/or Evaluation for Pre-aeration Pressure Adjustment and Addition of Alternative Stopper
At PLANT2, the lyophilized chamber pressure at pre-aeration step was adjusted to provide the optimal WFI suction volume during product reconstitution.
In addition, the alternative stopper was introduced to DP201 primary container closure system to improve the vial sticking during lyophilization process.
In order to apply these changes to DP201 drug product manufacturing process, a confirmatory run was undertaken at PLANT2 using a single commercial scale DP201 drug product batch.
As the change in lyophilization cycle and primary container closure system would not be expected to impact on the processes before the lyophilization, only the validation results from lyophilization, capping and visual inspection are presented in this section.
The confirmatory run at PLANT2 has been performed to cover the overall DP201 drug product manufacturing process
No significant deviations had occurred during the confirmatory run.
Two types of rubber stoppers (with Teflon coating and without Teflon coating) are used.
The difference between the 2 types of stopper is in the dimension (a slight difference in flange outside and plug outside diameter) and the presence or absence of Teflon (fluorocarbon) coating on the top of rubber stopper, which does not contact product directly.
The product contact material of stopper with Teflon coating is the same as used in the stopper without Teflon coating.
Therefore, the results from leachable assessment and compatibility during stability studies using rubber stopper without Teflon coating support the suitability for use of both types of stoppers.
The vacuum pressure decay test was performed as an in-process test for capping process.
The chamber pressure should be kept within 100-125 μbar during 1 minute for the 1st pressure rise test.
Translation - Spanish Validación del proceso y/o evaluación del ajuste de la presión de preaireación y agregado de un tapón alternativo
En la PLANT2, la presión en la cámara de liofilización en la etapa de preaireación fue ajustada para proporcionar el volumen de succión de agua para inyectables óptimo durante la reconstitución del producto.

Se utilizan dos tipos de tapones de goma (con revestimiento de Teflon y sin revestimiento de Teflon).
La diferencia entre los 2 tipos de tapón está en la dimensión (una leve diferencia en los diámetros externos del reborde y el émbolo) y en la presencia o ausencia de revestimiento de Teflon (fluorocarbono) del tapón de goma, que no contacta directamente con el producto.
El material de contacto con el producto del tapón con revestimiento de Teflon es el mismo que se usa en tapón sin revestimiento de Teflon.
Por lo tanto, los resultados de evaluación de lixiviables y compatibilidad durante los estudios de estabilidad usando tapones de goma con revestimiento de Teflon apoyan la idoneidad para el uso de ambos tipos de tapones.

Ha estado comiendo peor, aunque ha mejorado la ingesta en las últimas horas (pecho). No tos ni mucosidad nasal. No vómitos ni diarrea. No ambiente epidémico familiar.

EVOLUCIÓN
1. Sindrome febril en lactante de 2 meses con elevación de reactantes de fase aguda. Probable reacción vacunal.
Ingresa en UCI con monitorización de FC, FR y saturación de oxígeno. Se inicia tratamiento con cefotaxima y vancomicina a dosis meníngeas tras extracción de cultivos de sangre, orina y LCR y objetivar leve pleocitosis. Permanece estable desde el punto de vista respiratorio sin necesidades de oxígeno para mantener saturaciones normales. Diuresis en rango normal sin precisar soporte inotrópico. Desaparece la fiebre el mismo día de ingreso, mejoría franca del estado general a las 24 horas de ingreso.
Mantiene buena actitud alimentaria . Se suspende vancomicina tras comprobar negatividad de cultivo de LCR y queda con cefotaxima hasta comprobar negatividad de los dos hemocultivos.
Descenso de reactantes de fase aguda, se decide suspender cefotaxima tras 4 días de tratamiento . Permanece en observación 24 horas más sin presentar fiebre ni ningún otro síntoma.
Dados los antecedentes de vacuna de meningitis el día previo se asume el cuadro s. febril secundario a una reacción vacunal.
Se decide de acuerdo con la madre, el alta a domicilio con seguimiento en consultas.
Procedimientos : Monitorización , cuna térmica, ecografía cerebral, antibióticos, punción lumbar.

SARS Cov2 virus antigen in nasal swab: not detected.
CSF chemistry: no red blood cells, leukocytes 18/mm3, glucose 62 mg/dL, proteins 60.42 mg/dL.
CSF culture: negative
PCR detection of cerebrospinal fluid virus: negative for all virus tested.
PROGRESSION
1. Febrile syndrome in 2-month-old infant with elevated acute phase reactants. Probable vaccine reaction.
The infant is admitted to ICU with HR, RR, and oxygen saturation monitoring. Treatment with cefotaxime and vancomycin was started at meningeal doses after blood, urine and CSF cultures were collected and mild CSF pleocytosis was observed. He stays with stable respiration without oxygen requirements to maintain normal saturations. Urine output in normal range without requiring inotropic support. The fever disappears on the same day of admission, marked improvement in the general condition 24 hours after admission.
Maintains good food attitude. Vancomycin is suspended after checking CSF culture is negative and cefotaxime is continued until the two blood cultures are found negative.
Reduction of acute phase reactants, cefotaxime is suspended after 4 days of treatment. He remains under observation for an additional 24 hours without developing fever or any other symptoms.
Given the history of meningitis vaccine on the previous day, the febrile symptoms are considered secondary to a vaccine reaction.
In agreement with the mother, discharge to home with follow-up clinic visits is decided.
Procedures: Monitoring, thermal crib, brain ultrasound, antibiotics, lumbar puncture.

I have been working as a translator for 12 years. I was a lecturer and researcher at the
Genetics Department of the School of Medicine in Uruguay before. Also, I did a postdoctoral
research stay in Dublin, Ireland from 1994 to 1997, and acquired fluency in

always liked languages, I studied French, Portuguese, and English in secondary

For
completing my jobs I use Microsoft Office, online and offline CAT tools,
general and specialized dictionaries, the Cosnautas site with tools for medical
translators of Spanish into English. I research the important terms and
discuss them with colleagues, keeping the client’s confidentiality. I review
the translation once completed, and use Word grammar and spell-check.
I think a good
translator must be reliable, learned, meticulous, punctual, responsible, with
good command of the source language and native (or very fluent) in the target
language and the topics.
