Source text - Italian L’alveolite allergica estrinseca (anche nota come polmonite da ipersensibilità ), nella sua forma subacuta si caratterizza patologicamente per la presenza di polmonite interstiziale, focolai di bronchiolite e piccoli granulomi interstiziali mal formati, centrati sulle piccole vie aeree [24]. L’accentuazione bronchiolare/peribronchiolare di tali lesioni spiega la centrolobularità dei noduli visibili alla TC (Fig. 5a,b e 7a). L’ostruzione bronchiolare dovuta alla bronchiolite cellulata è responsabile della presenza di sporadiche aree di “ air trapping” lobulare, segno chiave per la diagnosi di questa malattia (Fig.7b). In circa l’ 80% dei pazienti, è inoltre presente un diffuso “vetro smerigliato” (ground-glass opacity) secondario a estesa polmonite interstiziale diffusa. L’eventuale combinazione di aspetti tipo vetro smerigliato, ridotta attenuazione polmonare e aree di parenchima normale possono realizzare un aspetto insolito definito segno della “coppa di testa” (head–cheese pattern), non patognomonico, ma suggestivo in paziente con anamnesi allergica [25,26].
Translation - English Extrinsic allergic alveolitis (also known as hypersensitivity pneumonitis) in its subacute form is pathologically characterised by the presence of interstitial pneumonia, foci of bronchiolitis and small, poorly formed interstitial granulomas centred along the small airways [24]. The bronchiolar/peribronchiolar concentration of these lesions explains the centrilobular nature of the nodules seen on CT (Figs. 5a,b and 7a). The bronchiolar obstruction due to cellular bronchiolitis is responsible for the presence of sporadic areas of lobular air trapping, a key sign in the diagnosis (Fig.7b). In approximately 80% of patients, there is also a diffuse ground-glass opacity secondary to extensive diffuse interstitial pneumonia. The possible combination of ground-glass opacity, reduced pulmonary attenuation and areas of normal parenchyma may give rise to an unusual appearance defined as head–cheese pattern, which is not pathognomonic but only suggestive of the disease in patients with a history of allergy [25, 26].

Nello specifico tali fattori si distinguono in: stabilità detrusoriale, integrità anatomica e neuro-vascolare dell’uretra, adeguata competenza e tono dello sfintere uretrale esterno, ottimale sinergia funzionale tra diaframma pelvico ed urogenitale ed, infine, la presenza di un adeguato clima estrogenico. L’incontinenza urinaria da stress (IUS) è definita dall’International Continence Society (ICS) come una condizione patologica caratterizzata, in assenza di contrattilità detrusoriale, dalla perdita involontaria di urina all’aumentare della pressione intraddominale. L’incidenza della IUS è in costante aumento, anche per l’aumentare dell’età media delle donne e, pertanto, dei fattori di rischio ad essa associati che esasperano una condizione clinica che influenza notevolmente lo svolgimento delle normali attività quotidiane, sia per gli episodi di incontinenza che per l’aumentata suscettibilità alle infezioni urinarie.
Translation - English The mechanism of urinary continence is ensured by the coexistence of several morphological and functional factors which guarantee regular micturition in its two phases of bladder filling and voiding. More specifically, these factors are: detrusor stability, anatomical and neurovascular integrity of the urethra, adequate competence and tone of the external urethral sphincter, optimal functional synergy between the pelvic and urogenital diaphragm, and the presence of adequate oestrogen levels. Stress urinary incontinence (SUI) is defined by the International Continence Society (ICS) as a pathological condition characterised by involuntary urine loss during a rise in intra-abdominal pressure in the absence of detrusor contraction. The incidence of SUI is rising in part as a result of the higher average age of the female population and of the associated risk factors that exasperate the condition, with a significant impact on daily life due to both incontinence episodes and greater susceptibility to urinary tract infections.
