Introduction
Thank you for taking the time to speak with me today.
My name is and I work with xxxx – an independent market research firm that works primarily in the biopharma industry.
Our research topic today concerns a rare genetic condition. Our discussion should take about one hour.
We are recording this conversation. This is so that I do not have to take extensive notes. I do want to assure you that your identity is confidential and no personally identifiable information will be included in the report that we provide to the pharmaceutical company sponsoring this research. We comply with all relevant codes of conduct and legislation for confidentiality in the conduct of market research. The sponsor of this research may be listening to this conversation (OR: observing from behind the glass) - however, as noted, your identity and institution are kept confidential. To maintain confidentiality, please do not use your name or the name of your institution in the course of this interview.
I’d also like you to know that you have the right to withdraw from the interview at any time in the process and may choose not to answer questions as you see fit.
Any questions before we begin?
I. Warm Up (5 Min)
Objective: Build rapport and understand practice environment, research interests (if relevant) and the types of conditions seen most often
• Tell me about your practice (type of setting, patient population served and types of conditions seen most often)
• Areas of particular interest in clinical practice and/or research
• When you think of rare diseases that you have come across in your practice, what specific diseases come to mind? What kind of challenges did you confront in dealing with these diseases? (Probe in particular for issues regarding diagnosis)
• When do you make the decision to refer out to another physician? Can you elaborate on any specific instances in which you referred out to a different specialist for a suspected rare condition? What kind of specialist did you refer out to and why?
II. Quick Exposure to Concepts and Overview of Individual Concepts (5 min)
Objective: Understand top-of-mind reactions and recall of what they see in the concepts based on “clutter reel” exposure
I have a number of boards/screens which introduce some information on a condition you may or may not be familiar with. To start off, I would like to have you take a quick look at each one. We’ll discuss each board/screen in more detail later in our discussion, so for now I’d just like you to look at each of these boards/screens briefly as I flip through them. As we go through them, it’d also be helpful if you kept in mind that what is shown here might appear in a number of different contexts such as journal ads, details aids etc. (Show one version of each concept in clutter for 5 seconds each, rotating in alternative versions of Doll and Babies)
• Thinking about all the boards/screens you just saw, what do you recall? Anything else? (Continue to probe until unproductive.)
• What do you remember seeing? What is your overall reaction to the elements you recall seeing?
- Probe for differences in reactions to words, images or ideas recalled
- Was there any one in particular that piqued your interest? If so, why?
• What one word would describe each of the concepts you saw?
• Are you familiar with AADC deficiency? What do you know about it? How is it different or similar to other rare conditions that you might deal with?
• Have you ever come across a patient with AADC deficiency? If so, tell me about your role in the management of this patient, challenges confronted and ultimate outcome.
III. Overview of Individual Concepts (15 min)
Objective: Understand initial reactions to concepts and variations (for Doll and Babies)
(Bring back primary concept versions one at a time. Use order of recall.)
• What is your reaction to this concept now that you have the chance to review it more closely?
• Does it grab your attention? If so, what specifically grabs your attention and why?
• What is the main message of this concept (visual and copy)? How relevant is that message to you?
• What actions, if any, would you take if you saw this concept?
• Does it make you want to learn more about AADCD? If you came across this somewhere, how likely would you be to go to the xxxx website? (5-point scale) Why?
• Does it raise your sense of the importance of thinking about AADCD as a potential diagnosis when the symptoms mentioned are present?
• Is there anything unclear or confusing?
• Doll (Specific Probes)
o The symptom of hypotonia is emphasized. What are your thoughts on this? (Importance relative to “developmental delay”)
o (Show alternative with same visual) Reactions to alternative headlines (Probe on urgency of “now more important than ever” and call out of “AADC deficiency” versus “genetic cause”
o (Show alternative visual) How would you describe the doll shown here compared to the one you first saw? Does the different visual give you any different feeling?
• Babies
o (Show alternative) Reactions to the alternative headlines (Probe to determine if one suggests greater urgency or importance in looking for AADCD)
What stood out to you about the visual representation associated with these headlines? Probe on single visually highlighted infant in concept.
• Genes
o Reaction to emphasis on genetic basis of disease and specific reference to genetic testing
• Of the alternative concepts you reviewed, is there one that you prefer? Why? Would this preferred concept, or any other, make you more likely to seek out more information on AADC-D?
IV. Whitepaper Review (30 min)
Objective: Explore reaction to more detailed discussion of AADC deficiency, as well as alternative headlines, copy and schematics
I’d now like to take you through some material which explores AADC deficiency in more detail. The material is divided up into different topic areas that I would like you to consider one at a time.
• Intro (Slides 1&2)
o (Show common body copy without headline or grey boxes)
o What are your reactions to the information shown here?
o Anything unclear?
o Anything you would disagree with? Probe in particular on conclusion: “confirming a diagnosis … is imperative”
o (Show pair of headlines [IH1&IH2] with body copy) Do you think one of these headlines would be more likely to urge you to find out more about AADC-D and genetic testing? Why? Probe on “knowing” versus “early” in headline.
o (Show pair of main body copy [IM1&IM2] with headlines and body copy) How about these ones, would one of these be more likely to urge you to find out more about AADC-D and genetic testing?
o (Show pair of boxes[IB1&IB2]) How about these ones, would one of these be more likely to urge you to find out more about AADC-D and genetic testing? Probe on relative emphasis on developmental delay versus specific symptoms as listed.
• About AADC Deficiency (Slides 3, 4, 5)
o (Show slide 3)
o What are your reactions to the information shown here?
o Anything unclear?
o Is there too much or too little detail here? What would you leave out? What would you want to add in?
o (Referring to the grey boxes in the schematic) Probe to see what value the grey-boxed information provides. Do you need this level of detail or would you prefer simpler grey boxes? Or no grey boxes at all?
o (Show slide 4)
o Given what you now know about the mechanism of AADCD, I’d like to gauge your preference for different headline options and see why you might think is most appropriate and why?
o (Show slide 5) Given what you now know about the mechanism of AADCD, I’d like to gauge your preference for different options of one specific section you saw on the earlier page, which one do you prefer and why? Probe on the following:
Appropriateness/preference between the terms, “neurometabolic disorder” versus “neurotransmitter disease” [AM2 and AM3]
Relative sense of urgency in diagnosing the condition
• Identifying AADC Deficiency (Slides 6, 7, 8)
o (Show common copy, table, graphic and chart[Slides 6&7])
o Do the “hallmark signs and symptoms” provide practical guidance for suspecting AADCD? Is any greater detail needed?
o Does additional information on secondary symptoms, severity and patient types add useful information?
o Is there anything that is not clear?
o Based on what you’ve seen here, what would be the “triggers” that would be most likely to make you suspect AADCD in a patient?
o (Show alternative headlines [Slide 8]) Do you have a preference for one headline over the others? Why?
• Diagnostic Pathway (Slide 9)
o Probe for degree of agreement with diagnostic pathway, need for additional clarification
o Willingness/ability to test for AADCD as described here.
• Misdiagnosis of AADC Deficiency (Slide 10, 11)
o (Show slide 10)
o Is it helpful for you to see the different conditions which AADC-D patients might commonly be misdiagnosed with? (Probe in particular for specific diseases listed under diagnostic pathway: cerebral palsy, floppy infant, etc.)
o Does the information here help in clarifying the differential diagnosis of AADCD? Are there questions you have that aren’t addressed here? If so, what?
o (Show slide 11 [MH1&MH2) Do you see a significant difference between this headline and the one on the last page, in terms of what they convey to you? How so? Probe on relative importance of “delaying treatment and proper management” versus “leaving families without definitive answers”
o How about the table on this page versus the table you saw on the last page [MT1&MT2], which of those is more useful for you and why?
• Diagnosing AADC Deficiency (Slide12, 13)
o (Show slide 12)
o If you were to summarize for a colleague how AADCD can be diagnosed, how would you sum it up?
o How would you characterize the need for genetic testing in diagnosing AADCD?
o If you were to encounter a patient in which you suspected AADCD, what would you do? Would you refer or run tests yourself?
If you did refer out, whom would you refer to and why?
If you decided not to refer out what tests would you run? Why these? Why not others?
o Is there any additional information you would want to see on confirming a diagnosis of AADCD?
o (Show slide 13 with alternative headlines[DH1, DH2, DH4]) Is there one of these headlines that you prefer for this section. Why? (Probe on underlying ideas of “readily available”, “patients suffering with unexplained developmental delay”, and “genetic testing”)
• Call to Action
o (Show Slide 14)
o Thinking about all the material we’ve gone through on AADCD, which of the following headlines best captures for you the importance of pursuing a clear diagnosis in suspect patients? Why? (Probe on “confirming” versus “identifying”, “now” versus “earlier”, and “life altering” versus “critical for patients and families”.
Is there one specific headline that would make it more likely for you to test for AADC-D? Why?
o (Show three “call to action” grey box statements together, alternating order[CB1, CB2, CB3]) Which of these statements comes closest to your take-away from everything you’ve seen today? Why? Probe on:
Role of genetic testing
Nuance in interpretation of “test for” versus “rule out” versus “consider… as part of your diagnostic approach”
V. Wrap Up (5 min)
Objective: Gain overall assessment of physician’s readiness to think about AADCD in suspect patients and pursue a definitive diagnosis
o Having reviewed all this detailed information about AADC-deficiency, do you have any additional feedback on the concepts we originally reviewed? Has it influenced how you perceive those concepts in any way?
In your mind, is there one concept that aligns the best with the information you just reviewed?
o Considering the information presented here, are there patients you’ve seen in the past year or so whom you now suspect of potentially suffering from AADCD?
o Had you been presented with this information at that time, would you have considered testing for AADCD? Why, why not?
o If you have a patient present with symptoms indicative of AADC-deficiency, would you pursue a diagnosis on your own or would you refer to another physician? Why?
o Did this information provide you with what you would need to know about what tests to order, or whom to best refer to?
o Overall, is there any information you think should have been presented here that was not?
o Imagine you are speaking to a colleague who is unaware of AADC-D. Given the material we just went over, what would you tell them about the condition, and about how you would approach assessing a patient who is exhibiting some of the signs/symptoms which were discussed.

Alguma pergunta antes de começarmos?
I. Aquecimento (5 minutos)
• Qual é a principal mensagem desse conceito (visual e texto)?
IV. Análise do white paper (30 min)
o (Mostrar texto, tabela, gráfico e quadro comuns [slides 6 e 7])

Papel do teste genético
