Depende do contexto.
Se a doença é tema ou objecto para debate, discussão, assunto, pode optar por "a matter of debate".
Se a doença origina ou coloca dúvidas, pode optar por "a matter of controversy".
Não creio que o facto de a palavra ser muito literal "penalize" a frase. A palavra existe em inglês. Era pior se traduzisse algum termo/palavra muito técnico literalmente. É a minha opinião.

Sorry, I messed up. It was meant to be translated from Portuguese to English. The appearance of the disease is a matter of debate, perhaps? "Controversy" just seems to be too literal. (I'm a amateur and got stuck on this one)
