Diagnosis:
Amputation of right big and second toe due to Diabetic foot ulcer Hypertension.
Medical History (Present & Past):
the above-mentioned patient is known case of diabetes since childhood, recently he developed hypertension
He has history of diabetic foot ulcer, for which he was admitted to Rashid hospital on 26/2/2016 and amputation of right big and second toe done with debridement of surrounding tissues.
He was under IV medication and regular wound care and discharged on 25/5/2016 to PHC to follow wound care as an outpatient.
He used to attend A1 Mamzar H C regularly for wound dressing and adjustment of diabetes treatment until today.

يبدو المريض بحالة طبيعية بالكشف على الأجهزة مع وجود جرح كبير مفتوح بالإضافة إلى العديد من الحطام الجلدي على القدم اليُمنى.

Subcontractor shall reimburse Contractor for any costs accumulated or penalties levied against Contractor due to the negligence or non-performance of the Subcontractor, and such costs or penalties may be deducted from the amount due to Subcontractor under this Agreement.
