NDA: Terminology problem

Hi,

I have to sign an NDA for a new client. At the end of the contract are the "translator's name:" and "address:" fields, but also "by:", "name:" and "title:"

Could anyone tell me what these last 3 are about ?

This is the signature field, where:
"by:" stands for "signed by:"
"name:" is where you indicate your name
and "title:" where you indicate your job title (translator, director, or the like).

Title could also be your title, eg Mrs, Mr as opposed to job title

Still unsure of this By, Name, Title, etc...

by: name of person who drafted it/read it/?
name:/title: name and title of signatory?

I guess, just leave it blank if those two are one and the same person.

Leftover from a company contract? Feb 7, 2014

It seems to me that the contract text may have been copied from a template for an agreement between two companies, where the Name and Address would be the company name and address, and the By and Title lines are for the company representative's name and job title who is signing the contract in the name of the company.
