I know this because I have many of these documents which I have had to obtain to renew my visa over the years. The documents I have are in both Croatian and English and this is the official translation.
It is not, however, a certificate of residence and cannot be use as proof of address, such is the bureaucracy in Croatia. If you want to get a certificate you have to go to the police and get a separate document (uvjerenje).
