Please refer to sentence:

After the injection you will be usually be free of pain for two to three hours as a result of the anaesthetic, but then the pain may return for 24 hours or so.

Does it mean that the pain may return after 24 hours or so or pain will remain for 24 hours or so. I shall be thankful if you could clarify the same.

You will receive an anesthetic (by injection) which will probably prevent pain, for 2-3 hs

After those 2-3 hours, the effect of the anesthetic will wear off (be gone).

Then, and for the next 24 hours, you could have pain again -because you are no longer protected by the anesthetic.

normally the period of pain would probably last around 26-27 hours (the first 2-3 are numbed by the anesthetic)

The pain will remain for 24 hours or so

After the anaesthic wears off, the pain may return and last 24 hours or so.

That's the way I read it.

not quite either. It means you may have pain on and off for 24 hours, or...

you may be in pain for the entire 24 hours after the anaesthetic wears off, or you may be pain-free for those 24 hours.

In other words, worst case, you will have pain for 24 hours straight after it wears off and best case you'll be pain-free even after it wears off

The pain may return in 2 to 3 hours, and then may last for up to 24 hours.

The patient appears to have had a procedure. The anesthetic should have taken care of the the pain during the procdure, and should take care of the pain for 2 to 3 hours immediately after the procedure. After the ansthesia wears off, the pain is expected to be felt, at which time other pain management interventions should be available, such as analgesic pills. The pain should gradually subside over a period of 24 hours.
