follow commands/on demand
In US medicine, this is usually translated as:
1) follow commands
and usually an explanatory detail is added
as in:
pt is not able to follow commands and raise his right arm on demand
OR
2) on demand
with an explanatory detail
as in:
pt is unable to raise his right arm on demand
and so on
I would not use "request" in the English
and would not use "darash" in the Hebrew

"Print out the Provisional Classification Form and guidelines and take it to your physiotherapist, occupational therapist or doctor. He or she will need to do a thorough assessment of you, as well as take footage of you doing some movements. It is also helpful to take footage of yourself doing your chosen sport (please see the information provided in the Classification Form for requested movements)."

"These may include psychological problems due to inability to properly execute the requested movements, and physical problems due to the stresses on the leg, pelvic girdle..."
