Russian to English Medical Medical: Health Care

Hello all

I'm translating a biography of Dr Buteyko, the Ukrainian doctor who invented a breathing technique that bears his name. He believed that conditions such as asthma and hypertension were caused by overbreathing (hyperventilation), and that patients must learn to breathe more shallowly. He attached great importance to the 'control pause' (the amount of time you can comfortably hold your breath for after breathing out) as a way of measuring your state of health.

In this section, a rival, Dr Dzagoyeva, is trying to take credit for the technique but has also introduced her own variations. A former patient of Buteyko's is telling the Doctor about it. He says:

"Предлагаю, мол, новый способ, обеспечивающий нормализацию легочной вентиляции... Это, дескать, достигается тем, что проводят дыхательные упражнения с задержкой дыхания на выдохе...Мол, такие задержки на выдохе обеспечивают должный для каждого больного объем дыхания... А затем Дзагоева... предлагает включать физические упражнения. Да не как-нибудь. А стоя с нагрузкой в период паузы на вдохе (!), что, по ее мнению, адаптирует аппарат дыхания больного к повышенным физическим нагрузкам..."

I'm confused by this section, in particular by "стоя с нагрузкой". Does this mean that Dr Dzagoyeva simply recommends standing while holding your breath on the out-breath (in a weightbearing position - ie with your legs bearing your weight, as in orthopaedics or radiography)? Or is she suggesting doing physical exercises while standing and holding your breath? Or doing physical exercises involving weights while holding the breath? (sounds distinctly unpleasant!).
