Well, I sometimes work with a reviewer of my own choice (who is a native speaker of the source language) and we operate in a spirit of teamwork, but I am not sure if that counts.

Not as a translator at any rate, which I think the asker is referring to. And I don't intend to either.
I'm not a team person and I'm glad that I don't have to pretend to be one anymore. I like to do my own thing, including accepting full responsibility for my work.

If you like the person you're working with, working on a team be great. I helped a colleague with a big translation of a book on the environment a few years ago. We were able to make suggestions and bounce ideas off each other. She is very reliable and easy to deal with, so I found it a great experience. On the other hand, working on a team with someone not of your own choosing or someone who is hard to reach or slow to get the job done could be very frustrating, so it's probably only a good ide... With a good mate of mine on a big project involving several manuals for a grinder for Mitsubishi Heavy Industries. Might I add that it was a 'grind'? Grooaan! 
He was originally a mechanical engineer who learnt Japanese and I was originally a Japanese graduate who came to Japan and learnt about mechanical engineering, primarily machine tools. We found that our skills mutually complemented each others. We had a blast, especially when we visited the training center and shop floor and talked with the Japanese engineers. Aaaah, those were the days! It's nice getting your hands grubby occasionally.
And, he's over here in Japan for a while so we'll be going out for a few jars soon. Kanpai!
Otherwise, I'm a pretty solitary animal like inkweaver who accepts 100% responsibility for work delivered. 
It takes a special animal to be a good translator, don't you think? And, we're always trying to get better and better.

I give thanks on a daily basis to have found the perfect job for me - which doesn't involve teamwork!!!! Accepting full responsibility is NOT a problem!

I worked in-house for 20 years in a team of Portuguese translators, but I presume this was not the intent of the question. As a freelance, I worked some years ago in a team with a medical doctor on the translation of an “Obstetrics & Gynaecology Handbook”. It was very hard work but quite pleasurable! Like Tim, I also have an arrangement with a trusted colleague where we provide each other with proofreading assistance.

No. I did it once in the past. I like to work as soloist. Maybe it confirms the prejudice....But I don't care about.

I'm with Kate on this. Till recently I worked with a team of other three translators on big projects (I was in charge). One of them was brilliant, very easy to deal with and we had this huge synergy. The second was pretty good too, the best thing was that he was always willing and capable to follow instructions and follow the team.
The third was the one we were stuck with: slow and incapable of making the proper choices when translating, but also refused sometimes to play along with the team. He was difficult to deal with and sometimes it required a lot of patience from the rest of us.
In spite of the above, in general we had a good time.
So, as Kate says, it works if you know who and what you're dealing with. I'm willing to do it again if the proper people are involved.

I've had to put together a small team myself on a couple of occasions, but I usually prefer to work on my own and I've never been coopted into a team by any agencies or similar.

Yeah, on some really large projects or involving languages I can't do. For example, I used to co-work with a German translator on some jobs, and I still sometimes help an Italian translator proof and edit her legal translations.
